Addressing health inequalities through inclusive design

In 2022 we partnered with Macmillan Cancer Support to evolve the way the organisation designs both for and with marginalised people living with cancer. In just five months we designed and tested a CoLab proposition that fundamentally shifted how Macmillan’s service design and innovation teams develop inclusive solutions.

The DENT BRINK MADE

In just five months we designed and tested a holistic ‘fatigue safety net’ proposition that was then piloted by healthcare staff across an NHS Trust in two hospitals. Along the way we changed how Macmillan’s service design and innovation teams develop inclusive solutions and codified the approach so that further innovations could be introduced and scaled across the organisation.

HOW WE DID IT

Systems thinking: In order to create solutions that could reach those who need it most, we pushed ourselves to think outside the hospital perspective and to consider non-traditional solutions. Understanding the wider context led to the discovery of opportunity areas and new ideas that would not have come our way otherwise.

Inclusive design: Part of this holistic approach meant involving people from all parts of the health and care system as well as those from marginalised groups. This involved going beyond clinicians and patients to learn from people living with cancer and their friends, families and community pharmacists, complementary therapists etc; as well as those from a wide range of ethnic and socio-economic groups.

Frontline innovation: We took our ideas into the real world by partnering early on in the design process with a NHS Trust to build and deliver new services that would normally take many months to set up.

Organisational change: At every step we brought the team together to collect perspectives, provide feedback, and capture learnings. All that experience was captured in an Inclusive Design Playbook, which became Macmilan’s deep and detailed resource on how to run an inclusive design project.

The story in more detail

“The worst hangover in your life, when you’ve just run a marathon… and have the flu.”

This is how some people describe cancer-related fatigue.

Despite being very common, cancer-related fatigue can be hard to pin down. Caused either by the cancer itself or coming as a side effect of treatment, some people are not affected at all, while others can find themselves severely debilitated. Similarly, the issue can impact individuals for different amounts of time in different ways depending on a whole host of factors.

Macmillan Cancer Support has an ongoing focus on tackling health inequalities across the cancer pathway and the charity is actively addressing the systematic differences between groups that result in some being more likely to suffer worse health than others. This existing priority, combined with the fact that socioeconomic status is one of the most influential factors in clinical fatigue, led to the the development of the Cancer-related Fatigue CoLab; an open partnership with Brink that was established to change the way the organisation designs both for and with marginalised people living with cancer.

Essentially, Brink’s brief was to work with Macmillan over the course of 20 weeks to deliver support to a hard-to-reach group suffering with a complex and hard-to-define problem that has no ‘one size fits all’ solution. That support would have to be holistic, tap into existing provisions and be capable of reaching the people who need it most, within a system that is overloaded and strapped for resources.

Along the way we would document our learnings and insights, share those findings and gather feedback from across the network. From this process we created a fit-for-purpose and actionable ‘inclusive design ‘playbook’ that could serve to drive future innovations and organisational change.

From service design to systemic thinking

Our role was to work with Macmillan to evolve their service design to deliver a more inclusive solution. This would mean bringing a more systemic approach, challenging established outlooks and bringing about change in the teams’ mindsets, methods and mechanisms.

We started by defining our ‘desired future’, crowdsourcing an ideal vision of what it would take to tackle fatigue for those in marginalised communities. We did this by setting up a coalition from the get go, bringing together multi-disciplinary perspectives from across the system and the UK - from people with lived experience to clinicians, academics, complementary therapists, funders, and NHS trusts. The power of this group was to become a sounding board as we evolved our thinking, providing critical friendship at every stage (as opposed to the more traditional research approach, where results might only be shared at the very end of a project). We then prioritised the resulting themes, which covered a multitude of perspectives and contexts. Off the back of this we were able to build a systemic view across what we called ‘The 7 Ps: Healthcare professionals, provision and funding, places, policy, paths to scale, product and of course, those people living with cancer themselves.

This resulted in a clear view of who was delivering cancer services to whom, what products and services they were delivering, how they were funded, where interventions were delivered, and the broader policies and agendas that might impact them. Once we had this picture we were ready to begin generating ideas that could be implemented across the most impactful points of the system we had mapped.

This approach led to two of the key insights that would help drive the direction of the rest of the project. Firstly, that the greatest impact could come from interventions at the pre-treatment stage as well as post-treatment stage; and secondly, that this support could be created through non-clinical staff at home and in the community.

As both of these insights went against many of the previous assumptions and accepted thinking in this area, it was vital that we were able to go out and test these propositions with people with lived experience of cancer-related fatigue.

From theorising to front line innovation

We worked with Macmillan to build a view of the benefits of a ‘get real’ strategy and created confidence and trust through clear accountability, regular validation and learning sessions. This allowed the group to deliver a bold pitch to the NHS and create a partnership with the Trust in York & Scarborough that gave us direct access to patients, clinicians and non-clinicians; a process which, ordinarily, would have taken months to achieve.

Through this ‘do to learn, not learn to do’ strategy we were able to move beyond two-dimensional user research and hypothetical solutions and begin to see for ourselves what worked and what didn’t. For example, visiting local charities, pharmacies, libraries and community centres, both in east London and Yorkshire, allowed us to gather insights, gain new perspectives, challenge assumptions and build relationships. This systemic approach meant we were able to better understand which methods needed iterating or supporting and how they might impact other parts of the support network.

Supporting all of this was a series of regular stakeholder meetups that brought together key players from across the system to provide support, share feedback and create accountability across the project. Additionally, daily standups between Brink, Macmillan and NHS teams, created clarity and a sense of camaraderie. This was another paradigm shift for the Macmillan team, who were more accustomed to working within their own network. One of the most rewarding parts of this project was seeing the team pick up on the momentum and clarity engendered by this practice.

A holistic solution that is just the beginning

The final result at the end of the five months was ‘The Fatigue Safety Net’, a four-cornered adaptive, proactive network capable of supporting a person living with cancer at any given point in their journey.

The safety net celebrates and upskills non-clinical staff so they are able to support patients across the journey, recommends that fatigue prevention coaching services be delivered to patients in high street pharmacies, and calls for a redesign of cancer fatigue information and access to discounted or free resources to sit alongside a more guided conversation with non-clinical advisors. The final piece of the jigsaw is 1-1 advice for carers, providing key information about fatigue prevention so they can support their loved one in the run up to treatment.

Crucially, this was not an untested proposal based on assumptions and theoretical ‘user needs’. Each element was tested and iterated in the real world, with a variety of patients, and with the help of staff in the hospital and non-clinicians across the community (i.e. the people who would be delivering these services in the long term and who had existing relationships with patients, giving them unparalleled insights). This commitment to making things real in the testing stage was the first step to creating a sustainable model. As well as demonstrating to everyone what it would take to deliver a new, holistic network of support, it also fed directly into our examination of the political and commercial paths to scale.

The safety net was not the only output of the five months. At each step of the way we had captured insights, learnings and methods and codified them as a series of ‘plays’; practical actionable approaches to shifting mental models, creating skills and transforming day-to-day practises.

The resulting playbook was designed to be a living manual; a deep and detailed resource on how to introduce inclusive design and CoLab-style across other parts of Macmillan’s organisation. From defining an ideal future vision, to testing solutions in the real world, the playbook showed how it was possible to think differently about solving complex problems, and how innovative solutions could be quickly and effectively spun up and applied to generate real impact.

Project outcomes
